The Osaki Manhattan Duo Mech is built around two independent roller mechanisms where nearly every massage chair has one. A 4D mechanism works the back, varying speed, depth, and rhythm along the upper portion of an SL-shaped track, while a separate 3D mechanism works the glutes and hamstrings at the lower end of the track with a steady kneading motion. Because the two run at the same time, the back and lower body receive massage simultaneously rather than waiting for a single roller to travel between them. A body scan measures shoulder height and posture before each session and positions the rollers for users from 5 feet 2 to 6 feet 4.

Twenty-six air cells at the shoulders, arms, hips, calves, and feet apply compression in coordination with the rollers. The calf section uses eight of those cells for deep kneading, the foot section combines rollers with memory foam kneading that reaches the Achilles tendon, and acupressure nodes in the armrests align with the reflexology points of the palms. Carbon fiber heating warms three zones: the lumbar region, the seat, and the calves.

Sixteen auto programs cover full-body routines, deep tissue, stretching, and targeted care, and seven manual programs apply individual techniques to a chosen area. A sensor in the left armrest reads heart rate, blood oxygen, heart rate variability, and a fatigue index at the start of a session and adjusts the program to the reading; results can be reviewed in the Osaki app. Zero-gravity recline raises the legs to heart level, a space-saving track lets the chair recline fully with 5.9 inches of clearance behind it, and the footrest extends up to 10 inches to match leg length.

Controls are on a 7-inch touchscreen tablet, a quick-access panel on the side of the chair for adjustments mid-session, and built-in voice commands. The chair includes Bluetooth speakers, a wireless phone charging pad, a USB port, seven-color LED lighting, a removable pillow headrest, and wood-grain armrest panels.
